Scheduling Appointments                                        

Our office personnel try to schedule patients on an appointment basis at a time most convenient to you. Your child may be seen by any of our physicians, or you may request the physician of your choice when your appointment is made. Our goal is to see your child at the appointment time. We suggest arriving a few minutes early so that the chart, etc., can be made available. Please notify us of any change in address, phone number, insurance coverage, responsible party, etc. This enables our office to provide prompt, accurate service to you.

Immunizations

If your child needs to come in for immunizations only, you do not need an appointment. Please bring your child to the office Tuesday - Thursday between the hours of 7:30 - 11:30 a.m. or 1:00 - 4:00 p.m.

School Excuses

Excuses for school absences will be issued from our office only when we have chart documents of the child's illness.

Release of Information

To protect the privacy and confidentiality of our patients, we cannot release information concerning our patients over the telephone. To release medical information we must have a signed release form from the parents or guardian. We will forward this information promptly to the designated party. Our office will release a copy of your child's office medical information only with a signed authorization by the parent or legal guardian unless otherwise specified by law. Records are copied once a week and will be forwarded by mail to the party as requested.

Financial Policies

Payment is expected at the time of service; this includes co-payment amounts for office visits and lab work for ALL insurance carriers. If you are not sure of your co-payment, cost share, or deductible amount, a minimum of $10.00 is expected. We accept cash, check, MasterCard, and Visa. The person bringing in your child will be responsible for payment at the time of service, so you should make arrangements if someone else brings your child to the office. For income tax and insurance reimbursement, you will receive an itemized receipt (superbill) providing a description of the services rendered, diagnosis, charges, method and amount of payment.
